This form is used to notify the regulatory authority about a change of information at a residual service. Use it to notify a change of information regarding:
- the approved provider (National Law s173)
- the approved service (National Law s173)
- a nominated supervisor (National Law s56)
Before beginning the notification
- Applicant Declaration
All notifications require an Applicant Declaration, which you can download and complete in advance.
- Declaration of fitness and propriety
If you are changing information about an approved provider that involves appointment or removal of a person with management or control of the service, the individual(s) concerned must complete a 'Declaration of Fitness and Propriety'. You will need to download and complete the declaration before beginning the notification.
You may scan or photograph the completed forms to upload during the application.

When submitting this form, refer to your legal requirements under the Education and Early Childhood Services (Registration and Standards) Regulations 2025 (SA). Providing false or misleading information may attract a penalty.
