Ensure your application can progress

Please be mindful that applications need to be submitted with all relevant supporting documentation. Payment is also required for a provider or service approval application.

Incomplete applications cannot be assessed. Should your application be incomplete you will be advised that your application is invalid, and you will be required to submit the missing documentation within 14 days. If we do not receive the missing documentation within this timeframe, your application will be terminated.
