Application and notification forms: education and care services in scope of the National Law

(e.g. centre-based services, long day care, OSHC, preschool, family day care)

There are a number of application and notification forms that education and care service providers are required to complete and send to the Board to assess and process. Both application and notification forms are available to be submitted online through the National Quality Agenda IT System (NQA ITS). Application forms relate to provider approval, service approval and supervisor approval.

    • Provider approval – to apply to become an approved provider or amend existing provider status.
    • Service approval – to apply to operate a service or amend service details. Nominated supervisor forms, waiver forms and other forms related to the running of a service can also be found here.
    • Supervisor certificate approval – to apply to become a certified supervisor or amend certified supervisor status.

Notification forms relate to prescribed circumstances, under the National Law, when an approved provider must notify the Board as the Regulatory Authority.

    • Notification forms – to notify prescribed circumstances, for example incidents or serious incidents, complaints, change to information about an approved provider or change to a nominated supervisor, and change to information about a service. It is important to note that there are specified timeframes for the different notifications. Please see our Notifications fact sheet for a list of required notifications and timeframes.

Application forms: residual early childhood services

(e.g. occasional care, in-home care, rural and mobile care, family day care (not part of a scheme))

There are a number of application forms that residual early childhood service providers are required to complete and send to the Board to assess and process.

Application forms relate to provider approval, service approval and supervisor approval.

Notification forms for residual early childhood services have been developed to support requirements of the Education and Care Services National Law as it applies to residual early childhood services. The notification forms relate to prescribed circumstances, for example incidents or serious incidents, complaints, change to information about an approved provider or change to a nominated supervisor, and change to information about a service.
